Please quote your lowest price on the item/s listed below, subject to the Terms and Conditions stipulated herein and submit your quotation duly signed by your representative not later than May 15, 2023, 10:00 o’clock in the morning at BAC Office, RTRomualdez, Agusan del Norte
Opening of Price Quotation: May 15, 2023, 10:00 a.m. at BAC Office, RTR, Agusan del Norte
Supplier shall submit the following requirements:
a. Duly signed Request for Quotation. Prices shall be quoted in Philippine Peso.
b. Mayor’s/business permit
c. DTI/SEC/CDA Registration
d. Philgeps Registration
e. Income/business tax return
f. Omnibus Sworn Statement
All deliveries by suppliers shall be subject to inspection and acceptance by the end-user.
Items shall be delivered to Municipal Agriculturist Office , RTRomualdez, Agusan del Norte
The supplier who submitted the lowest calculated responsive quotation shall be awarded the Purchase Order after evaluation.
Delivery items shall be made within 7 calendar days from date of receipt of Purchase Order. Price validity shall be for a period of 90 calendar days.
